Answer as many or as few as you like. The more you answer, the more your DM has to work with.

Backstory & Origins

1. How did your character spend their childhood? Where did they grow up, and who were their childhood friends?
2. What relationship does your character have with their parents and siblings?
3. When they were a child, what did your character want to be when they grew up?
4. If your character wasn’t an adventurer, what livelihood would they lead?
5. How did your character learn the languages that they speak?
6. What does your character’s name represent to them? (Or: why as a player did you choose this name?)

Appearance & First Impressions

7. Describe your character’s current appearance: clothes, armor, notable scars and their stories, distinguishing features.
8. What does your character’s voice or speech sound like?
9. What would most people think when they first see your character?

Personality & Identity

10. What are your character’s core moral beliefs?
11. What would your character say their best trait is?
12. What is your character’s biggest flaw?
13. Does your character think more with their heart or their brain?
14. Does your character care about how they’re perceived by others? How do they change themselves to fit in?
15. Does your character consider themselves a hero, villain, or something else?
16. What is your character’s biggest pet peeve?
17. What is your character’s guiltiest pleasure?

Goals & Motivations

18. What is your character’s current goal, summed up in one sentence?
19. What, currently, is your character the most curious about?
20. Where in the world does your character most want to visit?
21. Does your character ever want to “settle down” with a spouse, children, house, etc.?
22. Where does your character see themselves in 20 years?
23. What does your character want to be remembered by?
24. If your character was granted a single use of Wish, what would they use it for?

Fears, Regrets & Secrets

25. What is your character’s greatest fear — deep, irrational?
26. What is your character’s most recent or frequent nightmare?
27. What is the biggest mistake your character has ever made, and would they change it if they could?
28. What memory does your character want to forget the most?
29. What secret is your character keeping?

Beliefs & Worldview

30. What deity, if any, does your character worship? What’s their opinion on other people’s worship?
31. What is your character’s opinion on nobility? On authority?
32. Does your character have any biases for or against certain species?
33. What is your character’s relationship with magic? Are they scared of it, wish to know more, indifferent?
34. What does your character think is the true meaning of life?
35. What is your character’s opinion on killing others?
36. How far is your character willing to go for the “greater good”? Do they believe in one at all?
37. What does your character hope for the afterlife?

Relationships & Loyalty

38. Has your character ever been in love?
39. What person does your character admire most?
40. Who is your character’s biggest rival?
41. Who does your character owe a debt to — a favor, money, or their life?
42. If someone your character trusts asked them to act against the party’s best interest, who would they side with?
43. What decision would the party have to make for your character to consider leaving the group?

44. What is your character’s favorite spell, weapon, combat maneuver, or skill?
45. Is there any particular weapon, item, or artifact your character longs to find?
